Below is the list of health forms used by Byron Public Schools.

Medication Authorization Form (Staff Administration)

Complete this form if your child needs a medication given during school hours. A health care provider's signature is required for prescription medications.

School Immunization Form

This form should be used when asked to submit your child's immunizations and a printed copy from your child's medical care provider is not available. The backside of this form contains information regarding objection and exemption from immunizations.

Student Health Plan Forms

Byron Public Schools is committed to providing safe, individualized care to students with special health considerations. Communication with the school nurse is essential in ensuring that your child's health needs are managed appropriately in the school setting.

The development and implementation of your student's health plan allows for proper nursing interventions, and for the sharing of necessary information regarding your child's safety while at school. All student health plans are renewed annually at the beginning of each school year and as needed in response to any change in student health status.
